Arizona Medical Marijuana Card

In order to be afforded legal protection of the Arizona Medical Marijuana law, qualified medical marijuana patients must register with the mandatory state medical marijuana program and possess a valid identification card by submitting a marijuana card application with the Arizona Department of Health Services. Card-carrying patients are authorized to obtain, possess or cultivate cannabis for medicinal use.

There are two types of Qualified Patients: Adult Patients and Patients Under the Age of 18. For patients under 18, the patient’s custodial parent or legal guardian must be designated as his or her caregiver. Adult patients do not need to designate a caregiver. As per state law, patients and registered caregivers, may possess more than 2.5 ounces of marijuana in a 14-day period, to be obtained from a registered nonprofit medical marijuana dispensary — Unless a patient lives more than 25 miles from the nearest dispensary, then the patient/caregiver may cultivate up to 12 marijuana plants in an enclosed, locked facility.

How to Get a Medical Marijuana Card in Arizona

To legally qualify for medical marijuana in Arizona, you must complete the entire ADHS application form for the state’s medical marijuana program – *Please Note: this application is ONLY available online. Please follow the directions, provided by the ADHS, on how to apply for a Arizona medical marijuana card:

  1. Patients are required to obtain legitimate medical records or documentation from their primary care physician describing the diagnosis of their qualifying condition – *Learn how to request your medical records
  2. To qualify, a patient needs to be diagnosed by a physician as having one of the following debilitating medical conditions that is specifically identified in the law:
    1. Cancer; glaucoma; or positive status for the human immunodeficiency virus; Hepatitis C; Amyotrophic lateral sclerosis (Lou Gehrig’s disease); Crohn’s disease; agitation of Alzheimer’s disease ; or the treatment of these conditions,
    2. Or, a chronic or debilitating disease or medical condition or its treatment that produces one or more of the following: cachexia or wasting syndrome; severe and chronic pain; severe nausea; seizures, including those characteristic of epilepsy; or severe and persistent muscle spasms, including those characteristic of multiple sclerosis.
    3. Post Traumatic Stress Disorder (PTSD), effective January 01, 2015. 
  3. Patients are required to obtain a signed Physician’s Statement from a physician, currently licensed to practice medicine in Arizona – *Find a certified medical marijuana physician in Arizona
  4. Upon receiving a signed physician’s statement, all patients are required to register with the Arizona Department of Health Services, Medical Marijuana Registry Program – registration is mandatory.
  5. When submitting an application, please ensure the following:
    1. Firstly, if you do not already have Adobe Reader, please download it here – you will NOT be able to print your application receipt without it.
    2. Secondly, collect all documentation required for the application process and follow all application instructions for Adult Patients, Patients Under 18, or Caregivers. Before beginning the application process, be sure you have the following items in digital format ready for upload:
      1. electronic copy of photo id; and
      2. current digital photograph; and
      3. electronic copy of physician’s certification form; and
      4. electronic copy of your attestation; and
      5. Visa or Mastercard for payment; and
      6. if applicable, SNAP documentation.
      7. verify all data is correct before saving each page of the application.
      8. complete and submit the entire application without closing the browser – *Please Note: you will NOT be able to save and resume your application later.
