Pre-Visit Requirements

  1. Must be at least 18 years of age. 
  2. Must be a legal resident of Florida and provide valid proof of residency i.e. Florida driver's license, state identification card. 
  3. Must have a qualifying medical condition approved by the Office of Medical Marijuana Use and provide medical records indicating an exact diagnosis. 
  4. Complete medical records release and send it to the doctor who has your records.
  5. Confirm that we have received medical records, or bring them to your appointment.

The office is in Palm Beach Co., near West Palm Beach, Boca Raton, Ft. Lauderdale, Boynton Beach, Hialeah, Pembroke Pines, and Wellington. Dr. Mark Paris graduated from Tulane University in 1970 and also attended the University of Tennessee in Memphis. In 1975, Dr. Paris completed Basic Science in Ophthalmology at the Harvard Program at the Mass Eye and Ear Infirmary in Boston and completed a Neuro Ophthalmology Fellowship at Wills Eye Hospital in Philadelphia in 1976 and has been a fellow of the American College of Surgeons since 1980. Dr. Paris has also received a Master of Public Health degree in Epidemiology and International Health at the University of Alabama. In 1996, the doctor obtained a diploma in Tropical Medicine at the Universidad Cayetano Heredia in Lima, Peru, while also doing research on infectious disease. Dr. Mark Paris has a wealth of experience as a medical consultant to the University of South Florida Malaria Vector Control Project funded by the B&M Gates Foundation in Cambodia and is an assistant professor at USF, as well as a clinical preceptor for Nurse Practitioners at NSU, Kaplan and South University. In addition, the doctor has notably given speeches at Yale, John Hopkins, Auburn, Vanderbilt and Florida Atlantic Universities, as well as serving as a speaker and HIV/AIDS consultant for Gilead and Abbvie. Dr. Paris aspires to combine his far-reaching medical knowledge and expertise in order to provide the highest level of care to all his patients.
