In order to be afforded legal protection of the Ohio Medical Marijuana law, qualified medical marijuana patients must register with the mandatory state medical marijuana program and possess a valid identification card by submitting a marijuana card application to the Ohio Department of Health Services.

As per state law, allows for a maximum of a 90-day supply of medical marijuana — the specifics of which have yet to be specifically defined, and will be outlined in the rulemaking process.

How to Get a Medical Marijuana Card in Ohio

To legally qualify for medical marijuana in Ohio, you must complete the entire Ohio Medical Marijuana Control Program application form for the state’s medical marijuana program –  Please Note: this application is ONLY available online. Please follow the directions, provided by the OMMCP, on how to apply for a Ohio medical marijuana card:

  1. Patients are required to obtain legitimate medical records or documentation from their primary care physician describing the diagnosis of their qualifying condition – *Learn how to request your medical records
  2. To qualify, a patient needs to be diagnosed by a physician as having one of the following debilitating medical conditions that is specifically identified in the law: Patients are required to obtain a signed Physician’s Statement from a physician, currently licensed to practice medicine in Ohio – *Find a certified medical marijuana physician in Ohio
  3. Upon receiving a signed physician’s statement, all patients are required to register with the Ohio Medical Marijuana Control Program – registration is mandatory.
  4. After receiving a recommendation from a certified physicians, patients will be entered into the registry by said physician. Patients will then receive an email with details on how to access the portal and complete their application. There is currently a $50 fee for patients and a $25 fee for caregivers.”
