  1. Must be 18+ years of age. Patients who are minors must be accompanied by a parent, guardian, or registered caregiver during their appointment.
  2. Proof of residency in Washington must be provided.
  3. Must be diagnosed with a qualifying medical condition and have records documenting the diagnosis
  4. Confirm that we have received medical records, or bring them to your appointment.
  5. Holistic Evaluations offers a discounted rate for military veterans. ID Proof must be provided.

There is some important information you will need to provide our medical team at Holistic Evaluations before you arrive for your appointment. Our physician will be conducting a review of your health information to determine medical cannabis could provide wellness benefits for your needs.

There are three forms you will be required to download and fill out and return before your appointment with our team at Holistic Evaluations in Tacoma, Washington:
