Pre-Visit Requirements
  1. Patients must have a valid form of identification.
  2. Patients must be a legal resident of Rhode Island and have a valid proof of residency.
  3. Patients must be aged eighteen (18) years or older. Patients under the age of eighteen must consent from their legal parent or guardian, who must also accompany them to the scheduled office visit.
  4. Be diagnosed with a qualifying medical approved by the Rhode Island Department of Health. Please bring copies to your scheduled appointment. To obtain your medical records, complete a medical records release, and send it to the primary care provider who has your records.
