Pre-Visit Requirements
  1. All patients must be 18 years or older. Minor patients who are inquiring about treatment, please call the office directly.
  2. Must be a legal resident of Ohio and be able to provide proof of residency in the form of a state-issued identification card or driver's license.
  3. Proof of diagnosis with a qualifying condition must be provided
  4. To obtain your medical records, complete a medical records release, and send it to the doctor who has your records.

What Kind of Health Conditions Qualify for the Ohio Medical Marijuana Card?

To qualify and become registered as a patient in the Ohio Medical Marijuana Control Program (OMMCP), the patient must have one or more of the qualifying health conditions that are regulated by the Ohio Department of Health and the OMMCP.
