Pre-Visit Requirements

Before you schedule your appointment with TimeWise Medical in Williston, North Dakota, we ask that patients verify that they have the documentation and information they need to complete the process. You must be twenty-one (21) years of age or older to apply for your medical marijuana card. We appreciate our veterans and offer a 10% discount for those that have served our country and shop a government ID for proof of service. 

To complete your health evaluation and the application to become a registered medical marijuana patient in North Dakota, you will need:

  • A passport-style photo (taken recently)
  • An email address 
  • A telephone number
  • State issued photo identification. This can be a driver’s license or a state-issued non-driver ID card
  • A least one year of medical records for review, including a formal diagnosis of your qualifying condition by a primary care provider (PCP). If you have radiology imaging specific to your diagnosis, please provide them to our physician for review.

After you have completed your health evaluation with TimeWise Medical, you will need to complete the patient registration process online. Information is uploaded through the Division of Medical Marijuana secure portal, called the BioTrackTHC system.
