Dr. Moots has provided healthcare in Northeast Missouri for the past 51 years. Dr. Moots is a patient advocate and makes each patient his top priority. Dr. Moots has experience reviewing qualifying health conditions and providing certifications.

Dr. Moots specialized his first 18 years in OB/GYN. Afterward he continued his Family Practice and even provided house calls. He has also treated many Vietnam Veterans for PTSD. Awards he has received includes GP Physician Awards, and Director of Medical Education for 5 years.

Medical Marijuana when used can assist patients in transitioning from traditional medication to medical marijuana. Some of the conditions Dr. Moots has helped patients with incude: pain, migraines, muscle tremors, neuropathy, cancer and inflammation.

We can help you to get certified for your Medical Marijuana card and we can help you with your annual recertification. If you are recertifying, please try to do so one month prior to your card's expiration.

Please be sure to upload your medical records for Dr. Moots to review. Dr. Moots also needs to know what current medications you are on, as well as any allergies. After he has reviewed the records, he will be able to determine if you will qualify for a Medical Marijuana Certification. After your appointment, if you are arriving at the office, we will give you a copy of your two page evaluation. If you are utilizing our telehealth services, we will email, fax or mail your certification to you as soon as your appointment is complete and you have paid for your services.

Once you receive your certification you must upload the paperwork to the state portal. The state will charge you an additional $25 for your card as well as a small processing fee. If you are wanting to also be a home cultivator, the fee is $100 from the state as well as a small processing fee. The State is taking about 2-3 weeks to process all applications. When you are approved they will email you your card.

If you do not want to upload your information into the State Portal, we can assist you for a fee.

Please be sure to have your State of Missouri Driver's License and a recent picture of you, with a white background, similar to a driver's license photo, readily available.

Dr. Moots Card Clinic - Dr. Ferrel Moots, D.O. offers Medical Marijuana evaluations as an alternate to traditional medicine. Dr. Moots can evaluate you to see if you would qualify under the Missouri State statues.

The qualifying conditions in the State of Missouri are:

  • Cancer​
  • Epilepsy
  • Glaucoma
  • Intractable migraines unresponsive to other treatment
  • A chronic medical condition that causes sever, persistent pain or persistent muscle spasms, including, but not limited to those associated with MS, Seizures,
  • Parkinson's disease, and Tourette's syndrome
  • Debilitating psychiatric disorders, including, but not limited to, post traumatic stress disoorder, if diagnosed by a state licensed psychiatrist, must list their name
  • HIV/AIDS
  • A chronic medical condition that is normally treated with prescription medication that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication
  • A terminal illness
  • In the professional judgement of a physician, any chronic, debilitating or other medical condition including, but not limited to, Hep C, ALS, Inflammatory Bowel Disease, Crohn's disease, Huntington's Disease, Autism, Neuropathies, Sickle Cell Anemia, Agitation of Alzheimer's disease, Cachexia and Wasting Syndrome
