Pre-Visit Requirements
  1. Patients must have a valid form of identification.
  2. Patients must be a legal resident of Missouri and have a valid proof of residency.
  3. Patients must be aged twenty-one (21) years of age or older. Patients under the age of 21-years must obtain consent from their legal parent or guardian, who must also accompany them to the scheduled office visit.
