Leafwell MD is a patient-focused medical facility that believes in patient dignity, individuality, and choice. Patients who qualify that are interested in pursuing medical marijuana as an alternative treatment method can now request an appointment with Leafwell MD to be evaluated for a medical marijuana card. During the evaluation with the doctor, patients will receive education on the various different forms of ingestion for medical marijuana, the differences between THC and CBD based products, and will have the opportunity to ask any questions they may have. The attending clinician will use their medical expertise and judgement to determine whether or not the patient will benefit from obtaining a medical marijuana card. All patients who are approved for treatment by Leafwell MD will be entered in the registry maintained by the Illinois Medical Marijuana Program. As a compassionate and patient-focused practice, Leafwell MD is proud to provide Veteran Discounts and financial assistance to those who qualify.

Pre-Visit Requirements
  1. Patients must have valid form of identification.
  2. Patients must be a legal resident of Illinois and have a valid proof of residency.
  3. Patients must be aged eighteen (18) years of age of older. Patients under the age of eighteen (18) must obtain consent from their legal parent or guardian, who must also accompany them to the scheduled office visit. 
  4. Be diagnosed with a qualifying medical approved by the IL Medical Cannabis Program. 
    • Have medical records as proof of your current medical condition. Please bring copies to your scheduled appointment.To obtain your medical records, complete a medical records release and send it to the doctor who has your records.
