Pre-Registration Requirements Before You Schedule Your MMJ Card Health Evaluation

In Florida, every patient who wishes to participate in the medical marijuana program must become registered with the Office of Medical Marijuana Use (OMMU). To receive a Florida medical marijuana card for doctor-supervised cannabis use, patients must complete health evaluation.

At your health evaluation with Total Holistic Center in Boca Raton, the physician will review some information. The purpose of the assessment is to ensure that medical cannabis may provide wellness benefits for your specific symptoms and conditions.

What will you need to provide before you attend your in-office appointment?

Pre-Visit Requirements
  1. For patients under the age of 18, a legal guardian or parent must accompany them to the scheduled in-office visit.
  2. Be a legal resident of Florida and have valid proof of residency i.e. driver's license, state identification card or a seasonal resident (any person who temporarily resides in the state Florida for at least 31 consecutive days each year).
  3. Must have medical records documenting your current medical condition or diagnosis.
