Pre-Visit Requirements
  1. Must be at least 18 years of age.
  2. Must be a legal permanent resident of Connecticut with valid proof of residence, such as a driver’s license or state identification card.
  3. Must have a qualifying medical condition as outlined by the Connecticut Medical Marijuana Program.
  4. Must have medical records showing your current medical diagnosis. You may simply bring in your most current medical records to the office visit OR send your records prior to your confirmed appointment via email.
  5. You must have a designated primary care physician (family doctor).

What to Expect During Your Medical Cannabis Health Evaluation

Before you can register online to receive your CT medical marijuana certificate, you are required to receive a medical evaluation with a certified practitioner. Connecticut Cannabis provides online secure telemedicine appointments for patients who want to access legalized medical cannabis for therapeutic use.

Because patients have diagnosed health conditions, it is legally required that every medical marijuana certificate holder in Connecticut receives ongoing doctor supervision. The purpose of the evaluation is to review your pre-existing health conditions, symptoms, prescription medications, and aggregate pain level.

When the cannabis health evaluation is completed, and if the patient is approved, the marijuana doctor will initiate the patient application with the Connecticut Department of Consumer Protection. The patient will be emailed to complete the registration process, and pay the required fee for registering.
