LionHeart Medical Marijuana Cards is a cannabis clinic located in Mesa, Arizona, serving the communities of Chandler, Tempe, Gilbert, and Sunlakes. LionHeart Clinic makes the process of obtaining a medical marijuana card quick and easy for patients.

What Happens After My Health Exam with LionHeart Medical Marijuana Cards?
The fee you pay our medical cannabis clinic is for your Medical Marijuana Certification and exam (if needed). The AZDHS also charges their own fee which is $150 or $75 if you receive SNAP EBT. Your MMJ card is valid for 2 years. The State Fee is paid by the patient when the doctor submits your application to the AZDHS on the day of your appointment, using a credit card authorization form.
All required files including Patient Attestation and Physician MMJ Certification forms, photo identification, and payment are sent to AZDHS online, through a secure and HIPAA-compliant server.
The patient file is then reviewed by the ADHS and approved or denied based on the health information and recommendation from our physician at LionHeart Medical Marijuana Cards.
