All patients must be over 18 years of age.
Must be a resident of Arkansas.
Must bring valid, unexpired, ID issued by the state of Arkansas.
Patients must be diagnosed with a qualifying medical condition approved by the Arkansas Department of Health (ADH).
Must have medical records documenting your current diagnosis.
Patient must meet one of the 18+ qualifying conditions in Arkansas.
