Pre-Visit Requirements
  1. Patients must have a valid form of identification.
  2. Patients must be a legal resident of Arkansas and have a valid proof of residency.
  3. Patients must be aged eighteen (18) years of age or older. Patients under the age of eighteen (18) must obtain consent from their legal parent or guardian, who must also accompany them to the scheduled office visit.
  4. Be diagnosed with a qualifying medical approved by the AR Medical Cannabis Program.
  5. Have medical records as proof of your current medical condition. Please bring copies to your scheduled appointment. To obtain your medical records, complete a medical records release, and send it to the doctor who has your records.

What are the Qualifying Health Conditions for Medical Marijuana in Arkansas?

The Arkansas Department of Health (DOH) issues medical marijuana registry cards to patients who meet the qualifying health conditions. Cards are also issued to patients who require the assistance of a caregiver, such as a parent or legal guardian. The caregiver is required to apply on behalf of the patient and can assist by purchasing and administering medical cannabis to individuals with severe health conditions or terminal illnesses.

To become a medical marijuana caregiver in Arkansas, the applicant must:

  • Have a clear legal abstract and no criminal record.
  • Verify that the individual you are caring for is disabled or a minor.
  • Be a resident of Arkansas with valid proof of residency and photo identification with Arkansas Driver’s License or the AR State ID card).
  • Be 21 years of age or older.
  • Not be a member of the Arkansas National Guard or United States Military.
